Background: World Health Organization considers the outbreak of coronavirus disease 2019 (COVID-19) as a global pandemic. The perforation of Tympanic Membrane (TMP), followed by remission of COVID-19, can be an uncommon presentation. Case Report: This report is an unusual case of COVID-19 in an elderly woman who did not have any past medical history. She was admitted to the hospital with an uncommon, but serious presentation of TMP after one week of remission from COVID-19 infection. Otoscopy of ear revealed clear external auditory canal and central perforation in the pars tensa with regular margins occupying the posteroinferior quadrant. Computed tomography scan (axial view) with thin cuts of the temporal bone revealed well pneumatized mastoid regions without any density or secretion and normal middle ears without any density or secretion. After 2 months, tympanoplasty without mastoidectomy was performed, the patient recovered fully, the TMP was healed, and an audiogram performed 2 months later was found to be normal. Conclusion: This case highlighted the importance of attention to uncommon presentations, such as otalgia, and a sensation of fullness in the ear that can be subsequent to bacterial superinfection after Covid-19 infection.

外傷や中耳炎などにより鼓膜に穿孔が生じると, 聴力閾値が上昇する。 鼓膜穿孔時の中耳挙動は, 鼓膜が正常な状態のそれと異なると考えられるが, その振動様式は明確となっていない。 そこで本研究では, 有限要素法 (Finite Element Method: FEM) により, 実際に近い鼓膜穿孔を有するヒトの中耳のモデル化を行った。 そして, このモデルを用い, 鼓膜穿孔を有する場合の鼓膜と耳小骨の振動挙動を解析した。 その結果, 鼓膜穿孔部位や穿孔の大きさにより, 蝸牛内音圧は最大で約15dB減少し, 聴力閾値も相応に上昇すると推察された。 これは, 鼓膜穿孔が生じると, 鼓膜の振動モードが変化し, それに伴い, 耳小骨の振動モードも変化し, その結果, 内耳蝸牛への中耳伝音効率がわずかに減少するためと考えられた。 従って, 聴力閾値が大きく変化する場合には, 内耳蝸牛での相殺効果や鼓膜穿孔以外の複合的要因が考えられた。

Tympanic membrane(TM) perforation is one of the most common causes of hearing impairment. Apart from conduction of sound waves across the middle ear, the tympanic membrane, also sub-serves a protective function to the middle ear cleft and round window niche. The aim of the study is to determine the effect of the site of the tympanic membrane perforation on hearing. Seventy-eight patients with perforated tympanic membranes were included in the study.They all have TM perforation due to recurrent or chronic otitis media. Patients with traumatic TM perforation were excluded from the study. There were 47 males (46.5%) and 54 females (53.5%). Twenty patients had left ear perforation, 35 patients had right ear perforation, and 23 patients had bilateral perforations. Each ear was taken as a case, so the total number of the perforated ear drums was 101. The age ranged from 13- 56 with the mean of age was (31.64 ± 13.249) years. The TM perforation was examined and photo image was taken with the aid of the endoscope. Controls were 55 patients with a mean age 30.73 ±12.09 (range 14-53) years. Male controls were 34 (61.8%), and 21 female controls (38.2%). There is a positive correlation between site of TM perforation and hearing level, air bone gap, low frequency hearing level and high frequency hearing level. As a conclusion, there is a positive relationship between the site of TM perforation and hearing level being more in the posterior-superior and inferior parts.
