Even with Cox proportional hazards regression analysis, it is dif ficult to “ correct”for such differences, and there were almost certainly also other differences in unmeasured cofounders. Second, we did not collect complete data regarding the etiology of the MR itself (papillary muscle dysfunction, leaflet malalignment due to LV dilation, papillary muscle rupture), which may also affect prognosis. Nonetheless, it would appear, based upon these data, that for who are in need of coronary revascularization, that the presence of moderate or severe MR should be considered a relative contraindication for PCI. In summary, the presence of MR in patients undergoing PCI significantly and rather dramatically decreases survival over 3 years in a graded fashion, especially for patients with ejection fractions <40%.
