A Data Base Form for Pediatric Practice

A data base form for the recording of information for pediatric practice is described. Its features include sections for anticipatory and safety guidance, a section for developmental data, and a problem oriented format, as well as the standard elements of a medical record system. This data base form has been effectively used by a five-pediatrician group for the past three years.
