CLINICAL RESEARCH ON TOURNIQUET STRATEGY IN TOTAL KNEE ARTHROPLASTY

Objective To observe the effects of different tourniquet strategy on the patients undergoing total knee arthroplasty(TKA). Methods 40 cases undergoing unilateral TKA were divided into two groups randomly.Treatment group(20 cases,selective tourniquet strategy) used pneumatic tourniquet from the application of polymethylmethacrylate(PMMA) to the end.Control group(20 cases,omnidistance tourniquet strategy) used pneumatic tourniquet from the beginning of the operation to the end.Length of procedure,blood loss during operation,the volume collected in the drains,dominant amount of bleeding were recorded,hemoglobin,erythrocytes,hematokrit,the circumference of lower extremity were also recorded in preoperative and postoperative 1,3,7 days. Results There were no significant difference in length of procedure,blood loss during operation,dominant amount of bleeding,hemoglobin,erythrocytes,hematokrit.Volume collected in the drains were significantly less in treatment group(p0.05).Compared with postoperative swelling,treatment group was significantly less than control group at 10cm above the patella(p0.05),and at 10 cm below the patella there was no significant difference. Conclusion The selective tourniquet strategy in total knee arthroplasty does not increase total blood loss and operative time,and can reduce postoperative bleeding,reduce postoperative swelling of thigh.
