Abordaje y prevención del comportamiento suicida en el sistema sanitario español: enfoque, límites y riesgos

espanolEn los ultimos anos la prevencion del suicidio se ha convertido en una prioridad en las politicas de salud de muchos paises de nuestro entorno. En Espana no existe un Plan Nacional de Prevencion del Suicidio. Es por ello, que los servicios de salud de casi todas las Comunidades Autonomas han incorporado estrategias, planes y/o protocolos de prevencion y tratamiento del suicidio dentro de sus lineas prioritarias de atencion. Cabe preguntarse cuales son las premisas que subyacen a estas estrategias y si es posible pensar el suicidio desde otros modelos alternativos. El objetivo de este articulo es describir y discutir, desde una perspectiva critica, pero basada en la literatura y la practica clinica, el abordaje tipo del riesgo suicida segun se aplica en el sistema sanitario espanol. Se concluye que dicho abordaje sigue un modelo basicamente biomedico, el cual puede ser no solo ineficaz sino incluso contraproducente si se concibe como primario y no como una rama mas del tronco psicosocial de la prevencion. EnglishIn recent years, suicide prevention has become a priority in the health policies of many countries. There is no National Suicide Prevention Plan in Spain. This is the reason why the health services of almost all the Autonomous Communities have incorporated suicide prevention and treatment strategies, plans and/or protocols within their priority areas of care. It is worth asking what are the premises underlying these strategies and whether it is possible to think about suicide from alternative models. The objective of this article is to describe and discuss, from a critical perspective but based on clinical practice, the standard approach of suicide risk as applied in the spanish healthcare system. It is concluded that this approach follows a basically biomedical model, which can be not only ineffective but even counterproductive if it is conceived as primary and not as another branch of the psychosocial trunk of prevention.
