Vancomycin: the need to suit serum concentrations in hemodialysis patients

Thevancomycindoseforhemodialysis(HD)patientsshouldbeadjustedbymonitoringdrugserum concentrations. However, this procedure is not available in most health services in Brazil, which usuallyadoptsprotocolsbasedonpublishedstudies.Thetrialsavailablearecontroversial,andseveralhavenotbeenconductedwithcurrentdialyzers.Thisstudyaimedatassessingthesuitabilityof vancomycinserumconcentrationsinHDpatientsatapublichospital.BloodsamplesofHDpatients werecollectedfromNovember2006toMay2007,attimeintervalsof48,96,120,or168hoursafter vancomycinadministration.Drugmeasurementwasperformedwithpolarizedlightimmunofluorescence.Approximately86%oftroughvancomycinserumconcentrationswerebelowther ecommendedvalue,indicatingexposuretosubtherapeuticdosesandahigherriskforselectingresistant microorganisms.
