DIAGNÓSTICO HISTEROSCÓPICO DE CARCINOMA ADENOESCAMOSO INSERTO EN UN PÓLIPO ENDOCERVICAL. REPORTE DE UN CASO

Objective: Describe the case of a patient with adenosquamous carcinoma insert in an endocervical polyp evidenced by hysteroscopy. Case carried out in the Hospital de Clinicas Caracas. Patient and method: There was evaluated a 41 years old patient who attended gynecological consultation due to intermittent bleeding in "coffee grounds" for 7-10 days after menstruation, during the 2 months prior consultation. It was performed a diagnostic hysteroscopy, evidencing endocervical and endometrial polyps, one of which had a whitish lesion at its apex of unusual features, and whose biopsy reported adenosquamous carcinoma inserted in a polyp, performing a laparoscopic total hysterectomyand bilateral pelvic lymphadenectomy. The patient was discharged at 16 hours postoperative. The anatomical-pathological study reported surgical margins free of neoplasia and no evidence of metastatic lymph nodes. Conclusion: Due to adenosquamous carcinoma in endocervicalpolyp is rare (5%) and the diagnosis tends to occur in more advanced stages, it emphasizes the importance of hysteroscopic evaluation in all those patients with endocervical or endometrial polyps and highlight also the importance of hysteroscopy-directed biopsy for all those lesions suggestive of malignancy.
