Transition of follow-up care to primary care physicians (PCPs) of breast cancer (BC) survivors: Barriers and patient satisfaction.

e20613 Background: The number ofBC survivorsis increasing due to changing demographics and improved outcomes, putting pressure on cancer programs. Cancer Care Ontario has released clinical guidelines endorsing the role of the PCPs in well follow-up care of BC survivors and calling on Regional Cancer Programs to implement new models of care. This study aims to assess (i) provider barriers for early transition of care and (ii) patient satisfaction with a pilot transfer process. Methods: A cross-sectional survey was sent to all oncologists (n = 23) and PCPs (n = 300) practicing in the Toronto South Central LHIN. Questions included practice patterns, attitudes toward early transfer (oncologists), experience with- and readiness for BC patients, and educational needs (PCPs).
