Pramipexole-induced obsessive-compulsive symptoms in a patient with Parkinson's disease and disgust sensitivity.

While literature supports the phenomenological links between Parkinson’s disease (PD), obsessive-compulsive disorder (OCD), and impulse control disorders, the neurobiological underpinnings have yet to be established. Parkinson’s disease and its treatment have been implicated in the development of impulsive behaviors, including hypersexuality and pathologic gambling. They have not, however, been reported to induce frank OCD. In this case, we present a patient with PD and heightened disgust sensitivity who developed pramipexole-induced contamination OCD. As disgust sensitivity is positively correlated with the contamination subtype of OCD, this case illustrates the value of screening for this trait when patients require dopamine agonist treatment.
