Influence of glyburide on the antidiuretic response induced by 1-deamino-8-D-arginine vasopressin (DDAVP) in patients with pituitary diabetes insipidus.

Abstract The diuretic action of glyburide was confirmed in six patients with pituitary diabetes insipidus while on ad-lib. fluid intake. The present studies also demonstrate a progressive decrease in diuretic action of glyburide after intravenous administration of 0.04 μg, 0.12 μg, and 2 μg DDAVP, respectively, with a complete lack of the diuretic action within 2 hr after 0.04 μg DDAVP, within 4 hr after 0.12 μg DDAVP, and within 12 hr after 2 μg DDAVP. On the other hand, the duration of antidiuretic action of DDAVP was shortened significantly by simultaneous treatment with glyburide. The results of this study are consistent with the hypothesis that glyburide competitively inhibits the peripheral action of DDAVP. In light of this hypothesis, the diuretic action of glyburide was mediated through the inhibition of the peripheral action of the residual antidiuretic hormone in patients with pituitary diabetes insipidus.
