Introduction: The term stereotactic surgery was established to define a procedure that reaches targets in the brain, by a suitable device, through mathematical calculations. The device requires installation of a stereotactic halo in the skull by screws, and a reference bar through computed tomography images (CT) to obtain the three-dimensional coordinates. Objectives: Develop a new location stereotactic apparatus without the need of fixating pins in the skull, extract three-dimensional coordinates (X, Y, Z) from the film tomography and demonstrate the accuracy of the new method. Material and Methods: This study includes four human skulls CT with six targets within each. The coordinates extracted weretransferred to the new device. A statistical analysis to calculate the accuracy of the technique in 24 targets installed in the skulls was carried out. Results: The new device and technique presented an average error of 0.47 cm from the intended target. Conclusion: A new stereotactic device without attaching pins to the skull or reference bars and method of stereotactitc location capable of extracting film CT-dimensional coordinates (X, Y, Z) and to locate intracranial targets was developed.
