According to the Word Health Organization (WHO) estimates there are 500,000 maternal deaths worldwide each year. 99 percent occur in developing countries. And one of the developing countries is Indonesia. Worldwide in 2009 there were 2.7 million cases of perineal rupture in mothers who gave birth. This figure is estimated to reach 6.3 million in 2050, in line with the increasing number of midwives who do not know midwifery care properly. (Hilmy, 2010). The purpose of this study is to describe the incidence of maternal factors with perineal rupture at the Medan Hajj Hospital in 2018. This study is descriptive using secondary data from medical records. The population in this study were all pregnant women who experienced perineal rupture at the Medan Hajj Hospital in 2018, as many as 53 people. The sampling method is the total sampling of all women giving birth with perineal rupture at the Medan Hajj Hospital in 2018. Research tools and instruments are using research sheets that are in accordance with the conceptual framework, data analysis is discussed in accordance with existing theories and literature. From the results of research that has been conducted by researchers on "the description of the incidence of maternal incidence with perineal rupture at the Medan Hajj Hospital in 2018 based on the age of the majority of the mother 20-35 years (92.4%), based on the majority of baby weight of 2,500-3,500 grams (73 , 6%), based on the majority classification of degree I (52.8%), based on the majority of the handling (86.8%). It is hoped that the next researchers will be able to conduct a broader study of case descriptions of mothers in labor with perineal rupture with more research variables. From the data obtained, it is therefore advisable for hospital agencies to further improve the quality of service by completing facilities and counseling about the causes of perineal rupture, as well as for researchers to better understand about the occurrence of perineal rupture.
