Clinical features and significant of the prompt brain CT scan following cerebrovascular stentinig

2008
Objective To evaluate the clinical fertures and significant of the prompt brain CT scan following cerebrovascular stenting. Methods Clinical data for brain CT scan following cerebrovascular stentinig of 102 patients with symptomatic verebrovascular stenosis were analysized retrospectly.Different types were also classified according to different clinical features.According to the brain CT finding, the patients were divided into 3 types.Type Ⅰ was diagnosed as normal or local large vessels enrichment.Type Ⅱ was characterized as enrichment in the lesions of cerebral infarction and Type Ⅲ was diagnosed as intracranial hemorrhage or SAH.Finally, the different treatment methods and clinical outcome were investigated. Results In 102 patients with symptomatic verebrovascular stenosis, intracranial cerebrovascular stenosis was 67 cases, and extracranial cerebrovascular stenting in 35 cases. In TypeⅠ,there were 82(80.4%)patients.There were 13(12.7%)cases in Type Ⅱ.There were 7 cases in Type Ⅲ(6.8%)after MCA stenting,2 case were cerebral hemorrhage,1 case of cerebral hemorrhage combined subarachnoid hemorrhage(SAH),and 4 case of SAH.Regular anticoagulation and antiplatelet therapy were uesd in TypeⅠand Type Ⅱ patients. In Type Ⅲ cases, hemorrhage was not increased in early rechecking CT,and regular anticoagulation and antiplatelet therapy were used 24h after stenting.TypeⅠand Type Ⅱ patients fully recovered without complication.5 cases of Type Ⅲ fully recovered without complication also.2 cases of Type Ⅲ were treated with operation and discharged with severity complication of hemiplegia. Conclusion It is very important to perform the prompt brain CT scan after the placement of cerebrovascular stent. Moreover, the better clinical outcomes were obtained after treatment timely according to different types.
