The case is reported on a 72 year-old woman with dyspnoea and chest pain. Diagnostic tests showed lung images with a high suspicion of malignancy and an endoluminal defect in the pulmonary artery. Anticoagulant treatment was initiated under the diagnosis of pulmonary embolism and probable lung cancer. Due to a lack of response to the treatment, an MRI and PET-CT were performed, providing the diagnosis of a fusocellular sarcoma with pulmonary metastasis. Endoluminal sarcoma is an uncommon condition and is frequently misdiagnosed due to the clinical and radiological similarity with pulmonary embolism. This condition must be suspected when faced with some radiological characteristics and the lack of response to anticoagulant treatment.
