Laparoscopic Management of Interstitial Pregnancy

Abstract Background: Interstitial pregnancies represent 2%–4% of ectopic pregnancies, and are particularly dangerous because of the risk of sudden rupture and hemorrhage. Instead of traditional managements represented by cornual resection or hysterectomy by laparotomy, the endoscopic surgery has been developed. We report a safe and effective endoscopic technique of interstitial ectopic pregnancies. Case: A patient aged 36 years presenting a voluminous embryonic interstitial pregnancy was managed laparoscopically by the endoloop ligature method. There was no active bleeding at the uterine horn after salpingectomy. At 18 months postoperatively, the patient underwent a myomectomy by laparoscopy without any significant adhesions or defects in the cornual area. Conclusions: This original laparoscopic technique allows the simple, rapid, reproducible, and efficacious management of late-stage interstitial pregnancies. (J GYNECOL SURG 26:175)
