TRAITEMENT CHIRURGICAL DU CARCINOME HÉPATOCELLULAIRE

Introduction: During the last two decades surgery of hepatocellular carcinoma showed a great advances. Authors repart here their experience in surgical resection of hepatocellular carcinoma: - Patients and methods : This is a retrospecticve study including 25 patients, 18 males and 7 females. Criteria of Barcelona liver cancer conference 2000 were used for the diagnosis and therapeutic indications. Hepatic function and tumour stage were evaluated respectively by Child - Pugh and Okuda systems. Surgical treatment was hepatic resection. The evaluation of results were based on operative morbidity and mortality as well as the apperance of recurrence. - Results : The medium age was 60 years. Cirrhosis was found in 23 cases. 22 liver resections were done, two arterial ligatures were performed and one abstention caused by preoperative haemorrhge. We have noted 55% morbidity dominated by ascites and 4% mortality. Five recurrents have been registred and only one had iterative resection. Survival rate was of 96% for one year and 70% for three years. - Conclusion : Sugical management of hepatocellular corcinoma is globally sufficient. A better management should be obtained by multidisciplinary consultation to improve early diagnosis and treatment.
