Initially Diagnostically Overlooked Complicated Bilateral Sacroiliitis with Multilocular Abscesses

We present a striking picture of complicated bilateral sacroiliitis with multilocular abscesses in a 43-year-old lady with nonspecific symptoms. Prior to admission to our hospital, she had undergone computerized tomography scanning of the endocranium, abdomen, and the lesser pelvis—the findings were normal. We performed magnetic resonance imaging of the spine, which revealed multiple abscesses around the sacroiliac joints, in the iliacus muscle—bilaterally, in the erector spinae muscle, and in the left gluteus maximus and the left gluteus medius muscles. Sacroiliitis is a diagnostic challenge and is sometimes difficult to establish, as its symptoms are similar to other sources of back pain.
