Stroke Risk Factors and Ischemic Subtypes in a Multiethnic Population in Central and East Brooklyn (P07.050)

2012
Objective: To study stroke risk factors and ischemic subtypes in this understudied population. Background There has been an influx of non-Hispanic Caribbean immigrants to Central and East Brooklyn (>70% of the population). However, the data regarding the characteristics and risk factors after stroke are lacking. Design/Methods: In a prospective and hospital-based study the socio-demographic, clinical, radiological and laboratory information of patients presenting with acute stroke like symptoms at the University Hospital of Brooklyn are recorded. This hospital serves the Flatbush (central and east) region of Brooklyn. Along with standard risk factors, body mass index and estimated glomerular filtration rate (eGFR) are also recorded. Ischemic stroke subtypes are classified by using the Trial of Org 10172 Acute Stroke Treatment classification, modified to incorporate information on intracranial atherosclerosis. Results: Of 160 patients enrolled between June 2010 and July 2011, 109 had ischemic stroke, 24 TIA, and 27 had stroke mimics. Of the 133 patients with ischemic stroke and TIA, the mean age was 65 years (range, 27 to 99 years) and 56% were male. Race and ethnicity data revealed that 75% were Caribbean-American, 19% African-American, 4% Hispanic and 2% were Caucasians or Asian. 90% had hypertension and 40% had diabetes. More than half had never smoked. The majority of patients (69%) were overweight or obese. 43% had a mild decrease in eGFR (60-89 ml/min/1.73m2) and 31% had moderately decreased GFR ( Conclusions: Hypertension is the most significant risk factor of the stroke in this population with the majority of patients having evidence of chronic kidney disease at the time of stroke presentation. Intracranial atherosclerosis is twelve fold more common in extracranial atherosclerosis in this population. Disclosure: Dr. Panichpisal has nothing to disclose. Dr. Omoregie has nothing to disclose.
