e12571 Background: In the last 15 years at least 17 ASCO publications, 2 NCNN guidelines y 2 INC (Instituto Nacional del Cancer-Argentine) publications evaluated the necessity of genetic counseling while demonstrating the difficulty of implementing these recommendations. The following paper’s objective is to evaluate the feasibility of an automatic identification of high-risk patients (Pts) in order to refer them to genetic counseling. Methods: As of August 2012, all new IOHM Pts completed a Past Medical History Form (PMHF) with their personal and family medical history, The PMHF database was searched from its inception until August 1st 2014 with an algorithm designed to find at least one of the following criteria for genetic counseling: a) cancer diagnosis younger than 35, b) diagnosis of the same cancer in two first degree relatives or any cancer diagnosis in more than three first or second degree relatives.
