人工股関節全置換術における 1-deamino-8-D-arginine vasopressin の出血抑制効果

To evaluate the effects of 1-deamino-8-D-arginine vasopressin (DDAVP) on blood loss in surgery, we conducted a randomized trial of the drug on 42 patients undergoing total hip replacement. Twenty-two patients were designated to intravenously receive 0.3 μg/kg of DDAVP (DDAVP group), and the rest 20 were to receive a placebo (control group).The drug significantly reduced mean blood loss by 36.2% (from 1099.5±713.8ml in control group to 701.4±263.7ml in DDAVP group) and volume of blood transfused by 39.6% (from 947.5±540.2ml in control group to 572.7±396.8ml in DDAVP group) during surgery. Other parameters such as urinary output, postoperative blood loss and etc were not significantly different in the two groups. There were no untoward effects of DDAVP.We conclude that the intravenous administration of DDAVP during surgery can be recommended to reduce blood loss significantly in the patients undergoing total hip replacement.
