A three-dimensional scattered radiation imaging device of the present invention comprises: a detection unit which includes a first detector for detecting the position and energy of radiation emitted from a radiation source and scattered from an object, a second detector for detecting the position and energy of radiation having passed through the first detector, and a third detector for detecting the position and energy of radiation having passed through the second detector; a signal processing unit for receiving information on the positions and energy of the radiation detected by the first detector, the second detector, and the third detector so as to obtain the position of the radiation source by a method of reversely tracking the incident direction of the radiation; and an image processing unit for receiving information from the signal processing unit and displaying the same as an image. The three-dimensional scattered radiation imaging device according to the present invention can measure a radiation emission position and a radiation dose distribution in real time by a detection method using a multiple scattering technique during radiotherapy.
