Deletion of AZFc region of the Y chromosome is the most frequent molecularly defined cause of spermatogenic failure. Severe spermatogenic compromise may be the result of a Y-chromosomal deletion of the AZFc region. Unfortunately, 25-30% of men who have nonobstructive azoospermia will suffer of germinal epithelium degradation such that they will have no spermatogenic ability a short time later, making ICSI an impossibility. The use of frozen–thawed testicular tissue intentionally retrieved at a time remote from any ICSI cycle may solve these problems. A case is reported in which a pregnancy was achieved by ICSI with testicular sperm extraction (TESE), using spermatozoa from an azoospermic man carrying a Y chromosomal microdeletion confined to the AZFc region who used their frozen- thawed testicular samples.
