Does emphysema coexistence with IPF change prognosis

Introduction:: Different results were reported on the effect of emphysema (CPFE) in idiopathic pulmonary fibrosis (IPF). In this sudy, we analysed IPF with or without emphysema. Methods:: 67 patients, diagnosed as IPF pathological and/or radiologically were included. Patients9 clinical features at the time of diagnosis were recorded. High-resolution CT scans were reviewed by an expert and pulmoner conus were measured in means of pulmonary hypertension. According to the presence of emphysema, patients were classified in 2 groups. Results:: 17 patients had emphysema (Group 1). Smoking history (p=0,003), male gender (p=0,002), pulmonary cysts (p=0,024) were significantly higher than IPF patients without emphysema (Group2). All patients9 median survival time was 30±3,6 months. Shorter survival was associated with male gender (p=0,005), presence of emphysema (p=0,023), and honeycombing (p=0,019). In Cox multivariate analysis none of those factors were significant. Conclusion: In our series;CPFE was mostly seen in male gender and smokers. Presence of emphysema and honeycombing in male gender were related with shorter survival.
