The use of free functioning innervated gracilis muscle has evolved to become an invaluable tool in the restoration of elbow flexion and prehension in patients undergoing reconstruction following brachial plexus injuries. The purpose of this chapter is to describe the harvest technique of the gracilis myocutaneous flap, commencing with a distal dissection.
