[P054] Treatment of mycosis fungoides with oral alitretinoin and phototherapy

A 44-year-old woman was given the diagnosis of a mycosis fungoides 5 years ago. She did not achieve remission although she had initially been referred to department of hematology for total skin electrobeam therapy. We first tried phototherapy (narrow band UVB). However, she underwent the course of repeated reactivation for 3 years. Moreover newly appearing eczematous lesions on hands and feet were revealed to be histologically consistent with T-cell lineage malignant lymphoma. We tried adding oral alitretinoin (30mg per day) but failed because of a moderate headache as side effect. One year after no remarkable clinical improvement was achieved and the face was also involved. We continued phototherapy with excimer laser treatment for face and extremities and finally retried oral alitretinoin (30mg per day). No side effect occurred. She has shown steady improvement for 10 months so far and pigmented patches are left. The patient remains stable with continuous alitretinoin therapy. Herein we present this case to suggest alitreinoin as a new treatment option for patients with mycosis fungoides which do not respond to other treatments. Of course, further studies are necessary to verify the positive results. Alitretinoin is off label in mycosis fungoides and not many reports exist that alitretinoin has successfully treated patients with mycosis fungoides.
