Способ оперативного доступа к аневризмам торакоабдоминального отдела аорты

The description and the objective assessment of new surgical access to thoracoabdominal aortic aneurysms is provided in the article. Prescribed method allows to lower essentially level of a surgical trauma of the approach and to keep optimum parameters of visualization and a manipulation on each of objects of an intervention. The objective is solved by separation from a breast bone of the II-VII ribs, performance of a left thoracotomy to the III intercostal space, pericardium mobilization from a diaphragm, performance a paramedianny laparotomy, a diaphragm section in the field of a pericardiac part. The developed access excludes a section the left latissimus muscle of a back, the left anterior serratus muscle, the left rectus muscle of abdomen, ribs, vessels and nerves of thoracal and abdominal walls, allows to minimize damage of fibers of a diaphragmal nerve, to reduce potential risk of development of serious complications on various terms after operation, to improve quality of life of patients.
