The carpal tunnel syndrome (CTS) is the most frequent compression neuropathy. Several studies have demonstrated the efficacy of the wrist splinting in relieving the CTS symptoms. It has been verified that patients with CTS have an elevated intracanal pressure at rest and the splints in flexion and extension increase the pressure from three to six times that found in the neutral position. Immobilization of the wrists in the neutral position maximizes the space available in the carpal tunnel, minimizes the compression of the median nerve and provides greater symptomatic relief than splints in flexion or extension. This work has aimed to perform a bibliographic review on the treatment of the carpal tunnel syndrome using immobilization with splints. After this bibliographic review, the authors of this article suggest that the wrist should be immobilized in neutral position, with a perforated thermoplastic splint in form of cubital canal to be used during the day and night, that allows for the functionality of the hand (work activities and daily life activities) while relieving the symptoms.
