In this report, an extremely rare case of an inflamed appendix incarcerated in an abdominal hernia in a 53-year-old female after a gynecological procedure performed via a Pfannenstiel’s incision is presented. The diagnosis was reached intraoperatively, despite preoperative examination with ultrasonography. A typical appendectomy and simple sutured closure were performed. Authors’ case is discussed with the only such occurrence previously reported, as well as the more frequently encountered Amyand’s and de Garangeot’s hernias.
