D‐Tryptophan‐6‐LH‐RH‐at Low Doses in the Treatment of Male Subfertility

Summary The aim of this trial was to evaluate the effect of D-TRP-6-LH-RH in patients with idiopathic normogonadotropic oligoasthenozoospermia (I.N.O.). The LH and FSH response to LH-RH before and during treatment was also studied. Seven patients (age 27 to 32 years) with long standing infertility were incorporated. All of them were considered to have I.N.O. on the basis of at least three spermatograms and absence of evidence of other diseases. All patients were treated with D-TRP-6-LH-RH (2μg i.m. every two days) during 90 days. Control spermatograms were performed at monthly intervals during and after treatment. The responses of LH and FSH to 50 μg i.v. LH-RH were studied before and after 90 days of treatment. Five of the patients achieved a normalization of the concentration of spermatozoa per ml and of the percentage of forwardly progressive spermatozoa. None of the patients showed inhibition of the response to LH and FSH to LH-RH during treatment. These results differ from others in which larger doses of D-TRP-6-LH-RH determined an inhibition of the pituitary response to LH-RH and an impairment of the spermogram. D-Tryptophan-6-LH-RH in kleinen Dosen zur Behandlung der Subfertilitat des Mannes Zusammenfassung Diese Untersuchung soll die Wirkung von D-TRP-6-LH-RH auf Patienten mit einer idiopathischen normogonadotropen Oligoasthenozoospermie (I.N.O.) zeigen. Es wurde ebenso die Reaktion von LH und FSH auf LH-RH vor und nach der Applikation untersucht. Sieben Patienten im Alter von 27 bis 32 Jahren mit langer bestehender Infertilitat waren in diesem Kollektiv. Alle von ihnen hatten aufgrund der letzten 3 Spermiogramme eine I.N.O., eine andere Erkrankung lag nicht vor. Jeder Patient erhielt uber 90 Tage D-TRP-6-LH-RH (jeden 2. Tag 2 μg i.m.). Es wurden monatliche Kontrollspermiogramme vor und nach der Behandlung durchgefuhrt. Vor und nach der 90tagigen Behandlung wurde die Reaktion von LH und FSH auf 50 μg LH-RH i.v. gepruft. Bei 5 Patienten kam es zu einer Normalisierung der Spermatozoendichte und dem Prozentsatz der progressiv beweglichen Spermatozoen. Kein Patient zeigte eine verminderte Reaktion von LH und FSH auf LH-RH wahrend der Behandlung. Diese Ergebnisse weichen von anderen ab, bei denen mit hoheren Dosierungen eine Einschrankung der hypophysaren Antwort auf LH-RH nachgewiesen wurde und sich auch keine Anderung im Spermiogramm fand.
