Hormone replacement therapy and sexual response in postmenopause and geripause

Objective: Sexual responses in postmenopause and geripause (beyond the postmenopause) require clarification. Data from both mental and physical sex responses are needed to determine whether hormone replacement therapy (HRT) enhances sexuality. Methods: Thirty women treated with estrogen alone or combined estrogen and progestin and 30 parallel controls, ages 53–80 years, were evaluated over 3 months. Subjective information and objective data were obtained by routine questions and clinical examination. Results: Geripause transition was observed after age 64.4 years. Women treated with HRT were compared with controls: discomfort was less (P <0.01); less lubricant was necessary (P<0.001); fewer women discontinued vaginal intercourse (16%); desire and arousal continued to be present (P <0.02). Orgasmic responses through intercourse or masturbation were diverse (NS). Conclusions: This preliminary study indicated a positive effect by HRT on maintaining desire and arousal during geripause: prolongation of tissue protection was evident. Dose–response and time/duration correlations are needed.
