Laser peripheral iridectomy combined with laser peripheral iridoplasty for chronic angle-closure glaucoma with goniosynechia over 180°

Objective To explore the efficacy of laser peripheral iridectomy combined with laser peripheral iridoplasty for chronic angle closure glaucoma(CACG) with goniosynechia over 180°.Methods Fifty-six eyes of 43 cases of chronic angle closure glaucoma with goniosynechia over 180° were enrolled in the study.All cases were treated by laser peripheral iridectomy,which followed by laser peripheral iridoplasty 48 hours later.The intraocular pressure(IOP),chamber angle state,visual field before and after laser treatment were compared and analyzed.Results The follow-up was 18 months.After laser treatments,all cases had smaller range of goniosynechia and lower IOP,there was no change in visual field.At the last visit,there were 30 eyes (53.6.%) with IOP lower than 21 mmHg.The IOP of 19 eyes (33.9%) could be controlled by 1 ~ 3 kinds of antiglaucoma medication.The IOP of 7 eyes(12.5%) could not be controlled with two to three types of antiglaucoma medication,and they were then treated by filtering surgery.Conclusion laser peripheral iridectomy combined with laser peripheral iridoplasty has good clinical efficacy in the management of chronic angle closure glaucoma with goniosynechia over 180°,which reduces the proportion of filtering surgery.
