Ultrasound-guided Puncture of Complicated Ovarian Cysts: An Alternative to Surgery in Sub-

Objective: To propose ultrasound-guided puncture of complicated ovarian cysts as an alternative to surgery by demonstrating its safety and effectiveness in the short and medium term. Patients and methods: This is a Three-year prospective study carried out in the Teaching Hospital of Yopougon (AbidjanCote d’Ivoire). We performed a trans-parietal ultra-sound guided therapeutic puncture of complicated ovarian cysts in 7 outpatients. The post-procedure monitoring was carried out at D1, D8 and D30. Three patients were reviewed with a one-year follow-up. The aspirated fluid was examined in cytology. Results: The mean age of our patients was 26 years with an average parity of 2. The cysts were painful (2 cases), bleeding (3 cases), and in torsion (2 cases). They showed no organic character. Their average size was 74.3 mm. The removal of cysts was complete in 5 cases. In 2 cases, a second puncture was necessary. The aspirated fluid was hemorrhagic in 3 cases and serous in 4 cases. There was no complication at D1, D8 and D30. There was no cellular atypia in cytology. With a one-year follow-up, 3 women were reviewed in the context of prenatal check up. Conclusion: Ultrasound-guided puncture of ovarian functional cysts, even complicated, is an easy procedure to perform in a tropical environment. This is an effective and well tolerated technique, particularly the trans-vaginal puncture that is usually better adapted.
