Adrenal metastatic localisation from renal cell carcinoma is uncommon, with reported incidence being between 1.2 and 10% of operated cases and a clear tendency of involvement of the homolateral rather than the contralateral adrenal gland. The authors present 38 cases of adrenal metastasis from renal cell carcinoma, synchronous in 27 and metachronous in 11, with a minimum follow-up of 36 months after adrenalectomy. It is considered that in the absence of lymph node involvement or metastasis in other organs, adrenalectomy could cure the disease, especially in the case of metachronous adrenal metastasis.
