Longstanding tracheobronchomalacia: A forgotten cause of severe cough and its response to roflumilast

Abstract This case report describes a patient with moderately severe tracheobronchomalacia following mycoplasma pneumonia. The patient was considered to have obstructive lung disease despite no prior smoking or lung disease and failure to respond to standard treatment. The possibility of tracheal pathology causing cough and sputum was not considered in 23yrs confirming this to be a “forgotten zone”. The patient was treated with Roflumilast to reduce airway secretions with great success and the Immunology of Roflumilast is discussed.
