Rationale: Evidence has demonstrated that transitions in care are susceptible to errors; of specific interest are errors due to inaccuracy of medication. While integral to patient care, a precise medication reconciliation is not always obtained. This reveals the need for a systematic, clearly defined process to correctly obtain medication reconciliation upon admission to the hospital. Aim: Nurses and pediatric residents on the general pediatric floor and IMC will work together to complete accurate medication reconciliation (correct name, dosing, schedule, and medication route) for patients who are on 2 or more home medications.
