Source and Incidence of Errors in Laboratory Data Processing: II. Patient Identification Errors.

Abstract Patient identification errors, including both data missing and data wrong, affected one-quarter of laboratory requests received in three different hospital systems. Similar errors were observed in 40 other hospitals. These errors arise in the ordinary course of medical care outside the laboratory. Computer system verification and audit procedures suffice to correct them.
