Análisis de supervivencia en gastrectomía subtotal por cáncer gástrico. Concepción, Chile

Introduction: The Gastric Cancer represents a serious problem to the chilean public health, becauseit represents one of the principal cause sof death, being the most important malignant tumours. With local Disease, the five years survival on patients with gastric cancer it is 10% to 15%. The intention of our investigation is to analyze the associated factors in the survival of patients to subject subtotal gastrectomy for gastric cancer during 10 years at the Hospital Clinico Regional Concepcion. Material and Methods: Non-concurrent longitudinal study. By means of clinical stories, we studied 52 patients with Gastric Adenocarcinoma who were operate consecutively. The 100% of the patients were followed. The survival was analyze by Kaplan-Meier method and evaluate to 5 years. To compare survival curve, we used Log Rank Test (Mantel-Cox) or Breslow Test. We consider p<0,05 (C.I. 95%). Results: Men 25 (48.1%), women 27 (51.9%) ; average age 60.85 (DS=±13.9) years old; distal third 90.4%, middle third 9.6%; 39 advanced cancer, 13 early cancer; 10 (19.2%) patients has not lymphatic metastases; average postoperative hospital stay was 11 days (DS=±4.91); operative mortality 3.8%. Global 5 years survival was 48,3%; according to serous invasion: without invasion 70.0%, with invasion 32.2% (p=0.036); according to lymphovascular invasion: positive 30.6%, negative 71,6% (p=0.003); according to stage: I 72.2%, II 53.6%, III 40.6% and IV 25.0%, I v/s IV p=0.041; according postoperative complications: with complications 26.8%, without these 56,0% (p=0.017). Conclusions: The global survival was 48.3%, and its decrease is related with serious invasion, lymphovascular invasion positive, advanced stage and postoperative complications.
