Tumor de células granulares multifocal con afectación del tubo digestivo

2014
Granular cell tumors are uncommon and greatly distributed throughout the human organism. It can occur at any age but is most common in the fourth and fifth decades. There is a slight female predominance4. Its occurrence in the gastrointestinal tract is rare, accounting approximately for 8% of all tumors, among which the most common site is the esophagus, whereas gastric localization is very rare. Its behavior is usually benign, but cases of malignization have been described with distant metastasis or associated with other primary tumors. Histologically, these tumors consist of polygonal and fusiform cells disposed in compact "nests" and immunohistochemical staining for S-100 protein supports the proposed derivation from Schwann cells. A correct preoperative diagnosis of this tumor can only be made in 50% of all patients and it is always based on endoscopic ultrasonography and biopsy. Laparoscopic or conventional wedge resection represents the treatment of choice. We present the case of a patient with a multifocal GCT that compromises the vulva, breast, esophagus and stomach simultaneously, with a conservative stance with respect to the esophagus and a gastric resection was necessary to obtain histological confirmation.
