This narrative describes the care provided by a midwife nurse, trained in cognitive behavioral therapy to a woman with postpartum de-pression, with whom she establishes a therapeutic relationship. Prior written consent, therapeutic sessions were planned based on the theory of Aaron Beck, and during them, it was achieved that the puerpera recognized and reflected on a paper their negative thoughts, recognized the connections between thought, affection and behavior, examined the evidences that they are in favor or against their thoughts, correct their distortions, replace negative thoughts with more objective interpretations and learn to identify the conditioning schemes and autoschemes, which predispose to distort their experiences. The training and sensitivity of the midwife, allowed her to at-tend the puerpera, and recognize that beyond the consultation for mastitis, her initial demand, there was a depression, before which she had to act.
