114 NHS Lanarkshire lung cancer nursing team: patient referral guidelines

The Project Leads provided education sessions on the aims of Gold Standard Framework and raising awareness of End of Life issues for all levels of staff within the Trust. They focused on GSF 3 steps identifying patients who may be in their last year of life, assess current and future clinical and personal needs and develop an action plan of care. Results: • All Lung Cancer Patients diagnosed who the team suspected to be in their last year of life were identified at MDT. • GSF registration form completed and patient would be added to electronic locality based GSF register. Conclusions: • Improved Lung Cancer Team communication with patients and carers around End of Life Care. • Increased the Lung cancer team knowledge and confidence in End of Life care • An audit is underway to identify what impact the project has had on the Lung Cancer Patients diagnosed in 2010 and whether it has demonstrated an increase in use of end of life tools for this patient group.
