The osmolality of synovial fluid was measured in the knees of 15 rheumatoid, 15 osteoarthritic, 15 ligament injured patients and 18 adults with symptomless knee. Blood serum osmolality was also measured. There was no significant age related changes in the osmolality. Synovial fluid osmo- lality in pathologic conditions-osteoarthritis (295.6 +- 14.7 mmol/kg), rheumatoid arthritis (293 0+- 7.2 mmol/kg), and ligament injury (293.7 +- 6.9 mmol/kg) was significantJy different from that in symptomless knee (328.9+- 41.3 mmol/kg). Although the results indicate significant difference in osmolality between diseased and normal knee, it appars we cannot assess whether the knee is diseased or normal only based on synovial fluid osmolality. Further study is needed to be determining the normal range of values for synovial fluid osmolality.
