The clinical case of a 23 year-old woman with carbon monoxide (CO) poisoning associated with rhabdomyolysis is presented. They were no signs of compartment syndrome, neither myocardial nor neurological involvement Rhabdomyolysis is of uncommon presentation in CO poisoning and its pathogenesis, discarded crush injuries, is unclear. Ten to 15% of the CO that enters to the organism acts in the extravascular space, altering the oxidative phosphorylation process because its union to other iron-binding protein like myoglobin, enzymes of the cytochromes family and the NADPH reductase. It has been referred that the mortality rate for CO poisoning has little correlation with the carboxyhemoglobin, therefore mitochondrial dysfunction produced by direct action of the gas has been proposed, added to those well-known alterations in oxygen delivery.
