SUMMARY The surgical treatment of the cancer on the lower third of the rectum is a challenge and the best way to proceed is still matter of controversy. Several surgical options with low morbidity such as transanal local excision or high morbidity such as ultralow anterior resection and abdominoperineal resection can be used, depending on tumor local factors, patient characteristics and surgical team experience. During the last decade, laparoscopic access was incorporated as a new surgical approach for colorectal cancer and it was recently validated as a safe oncologic option. We report a 51 years old woman in whom a laparoscopic ultralow anterior resection of the rectum for adenocarcinoma located at 4 cm from the anal verge, was performed. The surgical procedure lasted 335 minutes, no postoperative complications were observed and the patient was discharged on the fifth postoperative day. Eight weeks later, the patient was admitted to close the ileostomy.
