EVALUACION DE CAMBIO PSIQUICO EN UNA PACIENTE CON ATAQUE DE PANICO

El presente trabajo se inscribe en el marco de la investigacion:“Evaluacion de cambio psiquico en pacientes con ataque de panico que reciben tratamiento Psicoterapeutico de Orientacion Psicoanalitico en el ambito publico”. En una investigacion previa se contrastaron dos tipos de tratamiento de uso frecuente en el ataque de panico en el ambito publico: monoterapia: Psicoterapia de orientacion psicoanalitica (POP) vs. Tratamiento combinado (TC), (POP mas psicofarmacoterapia). Se observo que los pacientes del segundo grupo no presentaban diferencias estadisticamente significativas en cuanto a la reduccion sintomatica, con relacion a los pacientes del primer grupo. Con estas conclusiones se investiga una muestra de 20 pacientes que recibirian monoterapia (POP), la evolucion y evaluacion de cambio psiquico. Consideramos cambio psiquico a modificaciones dadas en niveles preconcientes-conscientes (funciones yoicas de realidad) y en niveles inconscientes (posicion subjetiva). En este trabajo, se presentan los objetivos de la investigacion y los resultados obtenidos del analisis de un caso, donde se ha administrado el Psicodiagnostico de Rorschach al inicio y al finalizar un tratamiento psicoterapeutico focalizado con orientacion psicoanalitica. En el Psicodiagnostico de Rorschach se aplicaron las escalas ESPA (Escala de Potencial Suicida para Adultos) y EFYR (Escala de Funciones Yoicas de Realidad), comparando ambos protocolos. Palabras clave Cambio Psiquico, Ataque De Panico -Psicoanalisis, Rorschach ABSTRACT PSYCHOLOGICAL ASSESSMENT OF CHANGES IN A PATIENT WITH PANIC ATTACK The present work is included in the following research framework: “Psychic Change in Panic Attack Patients receiving Psychotherapeutic Treatment of Psychoanalytic Orientation within the Public Health Sphere”, In a previous study, two different types of frequently used panic attack treatments have been contrasted within the public health sphere: monotherapy -Psychoanalytic oriented psychotherapy (POP) v. Combined Therapy (CT, or POP plus pharmacotherapy). It has been observed that the second group of patients did not present statistically significant differences related to symptomatic reduction and results support regarding the first group of patients. Based on these findings, it emerged a proposal to investigate the psychic change progress and assessment on a 20-patient sample that would be under monotherapy (POP) treatment.We considered psychic change as the given variations at preconsciousconscious levels (ego features of reality), as well as at unconscious levels (subjective position). This paper presents research objectives and case study results, where the Rorschach Psychodiagnostic Test has been performed both at the beginning and at the end of a psychotherapeutic treatment focused on a psychoanalytical orientation. The PSSA (Potential Suicide Scale for Adults, as translated from Spanish) and SEFR (Scale Ego Functions of Reality, as translated form Spanish) scales were applied to the aforementioned test by comparing both protocols.
