Intracranial tuberculoma in AIDS: neuroradiological findings

Tuberculous infection is common in the AIDS population in our country. During a 5-year period, 400 patients with AIDS were treated at our institution. Intracranial tuberculoma was demonstrated in 4 patients (1%). Diagnosis was by histological studies or therapeutic response. CT scans showed ring lesions with hypodense centre and ring enhancement. MIR studies showed lesions with prolongation of the T1 relaxation time; on T2-weighted sequences, a hypointense centre with hyperin-tense surrounding oedema was observed. Control MRI with gadolinium (Gd)-DTPA was performed in 1 patient, showing ring enhancement. Tuberculomas should be included in the differential diagnosis of intracranial space-occupying lesions in AIDS populations. Early diagnosis and prompt medical treatment are essential in reducing morbidity, as in our series.
