Entre o medo e o desejo de crescer

O titulo da obra de Manuela Fleming serve como inspiracao para este caso clinico que aborda a adolescencia, fase em que a descoberta do corpo e da identidade so podem ocorrer quando associadas a um simultâneo processo de individuacao. Quando o sistema familiar e constituido por limites pouco definidos entre os seus membros, nao estimulando a autonomia do individuo, esta criado terreno fertil para o aparecimento de psicopatologia. O presente trabalho pretende retratar o processo diagnostico e percurso terapeutico de um jovem com doenca mental, em regime de internamento hospitalar parcial. O tratamento e de modelo grupal com outros adolescentes, sendo complementado com uma abordagem individual e familiar. Foi estabelecido o diagnostico de Perturbacao dos Sintomas Somaticos e avaliado um padrao de interdependencia identitaria com a figura paterna. Houve melhorias clinicas significativas ao longo do tratamento, sem registo de alteracoes  psicofarmacologicas relevantes. Durante o processo terapeutico, o jovem foi experienciando um novo modelo relacional com os pares e adultos, tendo sido tambem simultaneamente criado um espaco para a familia ser ouvida, permitindo uma autonomizacao lenta e progressiva do jovem em relacao a familia e da familia em relacao ao jovem doente. O caso descrito pretende transpor para a pratica clinica os conceitos teoricos da relacao dinâmica entre o aparecimento de psicopatologia na infância e adolescencia e o funcionamento do sistema familiar. The title of the book of Manuela Fleming serves as inspiration for this case report, that addresses adolescence, a phase in which the discovery of the body and identity, can only occur when associated with a simultaneous process of individuation. When the family system has undefined limits between its members and does not stimulate the individual’s autonomy, there is fertile ground for the appearance of psychopathology. This work intends to portray the diagnostic and therapeutic process of a young man with mental illness, in partial hospitalization. The treatment is group orientated, with other adolescents, and complemented with an individual and familiar approach. The diagnosis of Somatic Symptom Disturbance was established, and was identified a pattern of identity interdependence with the paternal figure. There were significant clinical improvements throughout the treatment, without relevant psychopharmacological changes. During the therapeutic process, the patient experienced a new relational model with the pairs and adults and there was created a space for the family to be heard, allowing a slow and progressive autonomization of the adolescent in relation to the family and the family in relation to the young patient. The case described aims to transpose into clinical practice, the theoretical concepts of the dynamic relationship between the onset of psychopathology in childhood and adolescence and the functioning of the family system.
