Retrograde femoral nailing may offer advantages for the treatment of femoral fracture. Retrograde nailing has been used to achieve good results in the treatment of very difficult fractures as AO 3.3 and AO 3.2. Some unsolved concerns remain on risks of articular complications. We present some historical perspective, indications, contraindications, and notes of preoperative planning and technique.
