Observation on efficacy of GP neoadjuvant chemotherapy plus surgery in treatment of stage-III non-small cell lung cancer

2008
Objectives To explore the feasibility and toxicity of preoperative GP(gemeitabine and cisplatin) neoadjuvant chemotherapy in the treatment of stage-Ⅲnon-small cell lung cancer and to evaluate its effects on down-regulation of tumor stage, resection rate and survival rate. Methods Sixty-two patients with stage-Ⅲnon-small cell lung cancer were randomly divided into two groups.Thirty patients (preopera- tive neoadjuvant chemotherapy group, group A) were given 2 cycles of neoadjuvant chemotherapy before op- eration with the regimen of gemcitabine + cisplatin, and 32 were treated only with surgery (control group B ) without neoadjuvant chemotherapy. Results In group A,the tumor response to the induction chemothera- py was 80.0% (24/30),the rate of down-regulation of tumor stage was 56.7% (17/30).The resection rate was 92.9% (26/28) in group A and 87.5% (28/32) in group B,with no significant differences in the operative complication and mortality between the 2 groups (P0.05).The 1-,2- and 3-year survival rates were 89.3% (25/28),78.6% (22/28) and 64.3% (18/28) in group A,and 71.9% (23/32),53.1% (17/32) and 40.6% (13/32) in group B respectively. The long-term survival rate in group A was remark- ably higher than that in group B (P0.01).Conclusion The preoperative GP neoadjuvant chemothera- py is safe and effective. It is helpful to decrease in the tumor stage,to increase in the resection rate of the tumor, and improving the long-term survival rate and life quality of patients with stage-Ⅲnon-small cell lung
