不同剂量6%羟乙基淀粉130/0.4预先给药对大鼠内毒素性急性肾损伤的影响

Objective To investigate the effects of 6% hydoxyethyl starch 130/0.4 (6% HES 130/0.4) pretreatment on the endotoxin-induced acute renal injury and its possible mechanism. Methods Forty-eight male SD rats were randomly divided into 6 groups (n=8 each). Group C with saline control, group L,H1, H2 and H3 received intravenously saline 30 ml/kg, 6% HES 130/0.4 7.5,15 or 30 ml/kg 1 hour before lipopolysaccharide (LPS)administration and group H4 with 6% HES 130/0.4 control. Acute lung injury was induced by intravenous LPS 5 mg/kg. The animals were killed by blood-letting after blood gases were monitored. The kidneys were removed for microscopic examination. Serum tumor necrosis factor-α(TNF-α), blood urea nitrogen (BUN), creatinine (Cr), maleic dialdehyde (MDA) concentration and superoxide dismutase (SOD) activity were also measured. Results Compared with group C, the level of pH and PaO2 decreased and level of PaCO2, serum TNF-α, BUN, Cr, MDA concentration and SOD activity increased in group L, H1, H2 and H3 (P<0.05). Compared with group L, the level of PaO2 increased and the level of PaCO2 decreased in group H1, H2 and H3, the level of TNF-α and MDA decreased and the SOD activity increased in group H1 and H2, the level of BUN, Cr increased in group H3 (P<0.05). Compared with group H3, the level of BUN and Cr decreased in group H1 and H2, the level of TNF-α and MDA decreased and the SOD activity increased in group H2 (P<0.05). There were no significant differences among all the indexes between group C and group H4 (P>0.05). Conclusion Treatment with the dose of HES 130/0.4 (30 mL/kg) can not cause acute renal injury. Pretreatment with the dose of HES 130/0.4 (15, 7.5 ml/kg) can attenuate LPS-induced systematic inflammation, but not aggravate renal damage. The dose of 30 ml/kg might aggravate renal damage.
