Is it Time for Endobronchial Therapy to Replace Surgery in the Treatment of Typical Endobronchial Carcinoid

Aim: More and more studies are emerging which have shown very good results of carcinoid tumor treatment in the central respiratory tract. The aim of this study is to investigate the efficacy of cryoapproach in diagnosis,treatment of typical carcinoids and other benign tumors Methods: Patients were consulted at tertiary care between 2016-19.12 patients underwent chest X Ray,AngioCT scan,Flexibile bronchoscpy. All of them hadnt lymph node and bronchial wall infiltration.Patients underwent general anesthesia,bronchoflex tube was used for intubation through which we put the balloon for hemostasis by bronchoscop.We have made the diagnosis of all cases with endobronchial tumor by cryobiopsy,obtaining big specimens.After confirmation of benign origine of tumor the patients underwent devitalisation,debulking and we removed all mass by cryoapproach. Result:12 patient underwent endobronchial tratment with combination of cryotherapy-electrocautery.9 cases resulted typical carcinoid,2 hamartoma,1 tracheal papillomatosis.The patients were followed up with FBS every 3 months,FBS,CT after 6 months and each year. Median follow up was 27 months with FBS,CT. No one of our patients treated with cryotherapy have had bronchial stenosis, no one have had local or remote relapse.The main complication was hemorrage which is managed with balloone. Conclusion: Obtaininig large specimens through cryobiopsy ensure accurate diagnosis by well differentiating the typical from atypical tumor or benign nature of other tumors.According to these results and others studies,for treatment of typical carcinoid it is time to replace surgery with less invasive and safer procedure like endobronchial cryo approach.
